Office Policy

If you are a new patient, please arrive 10-15 minutes before your scheduled appointment time to complete any necessary paperwork. If using insurance, please bring your dental insurance card.
For returning patients, a yearly paperwork update is required. If you have any changes in your medication(s), please provide us with an updated list.
